Upon arrival to our office, you will be asked to fill out forms regarding your current medical condition and previous medical history. This is a necessary part of the evaluation and examination process. Also, it enables us to complete insurance claims on your behalf. If you would like to fill out these forms prior to your appointment and bring them with your, you may download them here.

Health insurance is filed as a courtesy by our staff. We ask that you please provide us with current and accurate information regarding your insurance coverage. We are participating providers for Medicare, Medicaid, as well as various HMOs and PPOs. All co-payments are expected at the time of service. Any outstanding balance that remains after payment from your insurance company is due within 30 days. We accept cash, checks, MasterCard and VISA.
