I am a new patient. Which forms should I complete?

The forms that need to be completed are the Demographic Form, General Consent for Treatment and the Medical Questionnaire. If you are a Medicare patient, you will also need to complete the Medicare Secondary Payer form. Please click here to download and print all forms.


How do I obtain test results?

You will be notified at the time of the test as to how long it should take for the results to come back. Typically test results are completed within 5-7 business days, however, some results may require more time.  If you have not heard anything within 5 business days, please feel free to call the office.


What should I do in case of emergency?

If you feel that the emergency is life threatening, please call 911. For other issues that are not life threatening, you may always call the office. Outside of normal business hours, an answering service will help to direct your call.


What should I bring to my appointment?

Please bring all of your medications, including supplements or a complete list (including strength and dosage) of current medications to every appointment. In addition, please bring your current insurance card and payment method for insurance co-pays and copies of pertinent medical records. Insurance Information


How do I request a prescription refill?

We ask that you request all refills at the time of your visit, however, if you need to request a refill between visits, please contact your pharmacy and ask them to send an electronic refill request. Refill requests will be processed within 72 business hours. Electronic refill requests are the fastest and most efficient way to help us take care of your refill request. In some cases, you may need to be seen in the office prior.


How can I reach my doctor’s nurse?

You may speak to your doctor’s nurse or medical assistant by calling the main number to the office where you were seen.  The staff member who answers the phone will route your call appropriately.

How can I have my medical records sent to North Carolina Heart & Vascular?

In order to have your records sent to North Carolina Heart & Vascular, please complete the Release of Information form and send it back to us via mail or fax. We will handle it from there. Please use the fax number of the office location in which you see your physician.
